By signing this Waiver and Release of Liability (Agreement), I waive and release The Grey Dog LLC, its agents, servants, employees, insurers, successors and assigns from any and all claims, demands, causes of action, damages or suits at law and equity of any kind, including but not limited to claims for personal injury, property damage, medical expenses, loss of services, on account of or in any way related to or growing out of my presence or involvement at the facility. This waiver and release is intended to and does release The Grey Dog LLC from any and all liability for damages or injuries on account of or in any way related to or growing out of my negligence, the negligence of third parties and The Grey Dog LLC's negligence. This is not intended to release The Grey Dog LLC from any liability resulting from their intentional conduct. I further covenant and agree not to institute any claims or legal action against The Grey Dog LLC for any claim released by this Agreement. I further agree that should any claim be made against The Grey Dog LLC in contravention of this Agreement, including but not limited to derivative claims, I will protect, defend and completely indemnity (reimburse) The Grey Dog LLC for any such claim and expenses including attorney’s fees and costs incurred by The Grey Dog LLC in defending themselves or security indemnity hereunder.

I understand that The Grey Dog LLC is not responsible for any lost, stolen, or damaged valuables or property.

I acknowledge that I have received and read a copy of the current rules and regulations governing the use of the facility. I agree that I will fully comply with all rules and regulations and with any amendments.

I ACKNOWLEDGE THE “ACTIVITY” OF AXE THROWING IS DANGEROUS. There are inherent known and unanticipated risks and dangers and/or hazards of participating in this ACTIVITY. I understand that I could suffer property damage, as well as minor injuries or serious injuries, including bruises, scrapes, cuts, broken bones, psychological injury, paralysis, or even death, and/or loss resulting from any of these. I understand, accept, and assume all risks of my participation. I have adequate medical coverage for any injury to myself and agree to bear the cost of such treatment of any injury. I do not have any medical condition which could interfere with my safe participation in this ACTIVITY. I acknowledge that the risks of the ACTIVITY can be greatly reduced by using common sense.

I UNDERSTAND that by executing this agreement, I am representing that I am 18 years of age or older, and that I have read, and understand, the agreement. FOR PARENTS/LEGAL GUARDIANS OF PARTICIPANTS OF MINORITY AGE (08-17) I CERTIFY THAT I, as a parent/legal guardian, do consent and agree to his/her release as provided above, and myself, my child, and our heir, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities, incident to a minor child’s environment, or participation at The Grey Dog LLC. As provided herein, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent provided by law.

I give The Grey Dog Permission/Consent to take video and or pictures of me for posting of Social Media and Advertisement purposes ONLY, not to be sold to or given to any other company or used for any other purposes.

I understand that there is a $100.00 or more penalty fee for any broken axes or damage to The Grey Dog Property.
